Report to Main Office for Registration Procedures and Packets of all required documentation.
Requirements for School Entry: (Immunizations)
- Complete Florida Certification of Immunization – Blue Card Form DH 680 – according to grade level
- State of Florida School Entry Health Exam – Yellow Form DH 3040 – (Health Exam cannot be older than 12 months)
- Tuberculosis Clinical Screening, PPD or Chest X-ray.
